I went in for my first three appointments for a thumb ligament injury over about six weeks with Ms. Tritthart. Although she was kind and listened closely to my history in the first session, on each of the three occasions, when physically assessing my hands' mobility she failed to assess my thumb's mobility in one respect and my hand's mobility in another. It took until the end of my third session for me to bring up my palm not wanting to sit flat on the table because of the natural cupping, and then, while Ms. Tritthart looked at the next patient who had just entered and began sending me off with my exercise prescription, she did not think to ask about my thumb's mobility backwards (ie: opposite of the opposability movements that she focused on) until I stopped her. She assessed this kind of movement for the first time, agreed this was an issue and gave me another exercise. When I stopped her for the second time and showed her how little backwards movement my injured thumb was capable of compared to my healthy thumb, she assessed me for this movement for the first time, agreed and gave me another exercise to correct it. I also believe the absence of identifying this problem sooner than the 6th week after meeting Ms. Tritthart was due to a failure to ask specific questions in follow-up appointments. Ms. Tritthart's questions of how I was able to use my thumb were not very specific; anyone can ask me "how is my thumb feeling?" or "how is its impairment affecting daily use?," but I go to a physiotherapist to hear that and more. I was not asked specific questions that I believe a hand therapist should know to ask after presumably working with many patients with similar problems. A thumb injury affects most ways one uses their hand. I could list any number of issues it causes me in daily life when asked. I needed to be asked questions that could expose common issues that come up with patients like myself, such as, for example, my hand naturally cupping due to stiffness from the injury, solely doing physiotherapy exercises to regain opposability for the thumb (ie: inward movements), and using the thumb for grabbing motions repetitively throughout my occupation. I think that not having a compensatory opposite motion in my suggested physiotherapy exercise regimen contributed to this additional issue arising. Absence of pain is not the only thing that matters - normal range of motion in all directions is the average patients' final goal. Hopefully, I have made a clear case for how I took two steps forward and one step back with the incomplete exercise regimen over these 6 weeks. I am not impressed with this and am going elsewhere. As a student, paying an average of $80-95/session and receiving incomplete assessment and recommendations is upsetting.
