★ ★ ★ ★ ★ We were sad to lose her as our doctor when we moved out of Burlington. She truly cares about her patients and she felt like an extended part of our family. Great doctor and an even more wonderful person. Her staff are an extention of her; warm and patient.

i have been coming to Dr.middlebrook since i was very little and even before her there was Dr.Prascovics the real (patch adams of all doctors) being with MiIddlebrook has changed my life in so many positive ways she is EVERYTHING you would want or need in a doctor and is highly knowledgable in the mental health field.
that being said her Reciptioinist Terry, has ruined my 19 years of coming here. She has lost my records from hospitals not once not twice but three times along with my blood work charts that i pay money to the hospital to send to her and also for her to send to other doctors, and not only mine but my mothers as well.
She has mis-schedueled and mislead many conversations that were to be forwarded to my Dr., what is required of her and where certain papers should be sent or kept on file or to other places. I have had so many issues with this woman that I now refuse to drive 2 hours just to come for my Doctors care no longer. Which is greatly dissipointing and sad dens me deeply because i feel my relationship with Dr.Middlebrook is one i will never feel with any other doctor,as kind and caring doctors like her are one in a million this day and age. Will hopefully return when word has come Terry has retired.
